Fahad Hashmi
Chief Executive Officer
Fahad Hashmi is a senior level executive with more than 12 years of experience in leading companies through rapid growth strategies, market expansions, transitional management, start-ups, and organization restructuring with a broad sector of finance, IT, and healthcare. He also has over a decade of experience in the financial industry, working for organizations like Citi Group, UBS, Morgan Stanley, and Fannie Mae. He acquired his MBA in 2009 from Cass Business School in London. As Chief Executive Officer has led healthcare organizations in digital healthcare, urgent care, laboratory, and behavioral health before leading Personic Health’s growth in mobile wound care. He also has significant experience in philanthropic projects serving the underserved. He brings his passion for organizational excellence with service to the organization.
Dr. Azmat Husain
Chief Strategy Officer
Dr. Azmat Husain serves as Chief Strategy Officer for Personic Health. He is an experienced physician executive having served in leadership roles in both hospital and physician practice management. He completed his medical training at the University of Maryland and residency in Emergency Medicine at the Thomas Jefferson University Hospital. He has served as Chairman of Emergency Medicine at Howard University Hospital and also served in executive leadership roles including as a CEO for both Adult and Children Hospitals. He also served on the Governor of Pennsylvania, Joshua Shapiro’s transition team for healthcare. Dr. Husain brings his healthcare executive and strategy leadership experience to Personic Health.
Dr. Shawn Naqvi
Chief Medical Officer
Dr. Shawn Naqvi serves as Chief Medical Officer for Personic Health. Dr. Naqvi has always had a passion for multidisciplinary care focused on the individual needs of patients. He attended George Mason University, Georgetown University, and Virginia Tech University locally before completing his Internal Medicine residency training in the Philadelphia area at Christiana Care Hospital. He is board certified in Internal Medicine. He also participates in medical education with medical students and resident physicians. Dr. Naqvi is passionate about underserved medicine and devoting his time to directorship roles in clinical practice improvements. He brings his experience as a clinical collaborator and ardent patient advocate to Personic Health.
Dr. Jose Rivera
Chief Wound Care Officer
Dr. Jose Rivera serves as Chief Wound Care Officer for Personic Health. Dr. Rivera has always had a strong passion for changing healthcare delivery for the better. He completed a Foot and Ankle Surgical Residency at West Los Angeles and Boston VA Healthcare Systems and went on to complete additional Foot and Ankle Surgical training at the University of Maryland Medical Center in Baltimore. Now, with over 18 years of experience in clinical practice, he is an exceptionally driven, helpful, and reliable director who strives to make a lasting impact on his patients’ health. He is one of the nation’s leading providers of advanced wound care in the home setting.
Nick Jagodzinski
Vice President of Clinical Quality
Nick Jagodzinski serves as the Vice President of Clinical Quality. Nick has been a nurse for over a decade with experience in leadership, business development, compliance, and patient care. With a master's degree in healthcare administration and a certification in wound care and ostomy, his experience allows him to focus on a variety of different aspects of the patient care continuum. He sits on the ad hoc test writing board and the board of directors for the National Alliance of Wound Care and Ostomy (NAWCO). As an Army combat veteran, he brings a high expectation of ensuring that patients receive evidence-based, best-practice, and quality care that falls within compliance and regulations.
Dr. Loren Nunley
Partner Provider
Dr. Nunley completed his internal medicine residency at Thomas Jefferson University and fellowship specialty training in Infectious Diseases at the Medical College of Wisconsin. He specializes in providing infection control and antimicrobial stewardship support. He is a member of the Infectious Diseases Society of America (IDSA) where he currently serves on the Inclusion, Diversity, Access, & Equity (IDA&E) Committee.
