How do I refer a patient?

When should I refer a patient?
You should refer any patient with an acute wound, a non-healing wound > 30 days duration, diabetic wound, pressure wound, traumatic wound, surgical wound, those being treated with negative pressure wound therapy (i.e. VAC), or those being treated in an SNF with a stage 2 wound or greater.
Can I schedule a same-day visit?
Depending on our availability, we may be able to schedule a same-day visit. Call us at (855) 479-4217 and we will be more than happy to help you schedule a visit for your loved one.
How often will you come to my home to take care of my wound?
Depending on the severity of your wound, we can set up a weekly, bi-weekly, or monthly wound care treatment plan. If you require daily care (ex. changed bandages, wound cleaning, etc.), we will schedule our support team to help you.
What diagnostics do you perform?
X-rays, ultrasound, wound cultures, and custom bracing.
Will you reach out to me before you come to my home?
Yes. We will always call you to schedule and confirm a specific date and time before we visit your home.
How many professionals will be in charge of my care?
Every patient will have an expert support team that is made up of their very own oversight physician, patient care coordinator, and medical assistant.
